Helping people with chronic conditions
Health Partners Group offers a chronic conditions coaching and support service for people with type 2 diabetes and weight issues/obesity. This strategic move marks an important milestone in our commitment to supporting the wellbeing of our clients through innovative health and wellbeing services.
The programmes were developed by renowned academics who devised market-leading, evidence-based health coaching programmes which have been peer reviewed in academic journals and are used and endorsed by the NHS and other leading healthcare groups. Within our Changing Health wellbeing services, the chronic conditions coaching and support programmes have developed to be as efficient and cost effective as possible without losing the human touch, which is so important for effective change. These services are not intended to replace treatment management by a primary care specialist, but instead to complement medical treatment.
Award-winning digital solutions
Our chronic conditions coaching and support service is a Turn Key digital health platform and behaviour change programme that helps people with type 2 diabetes and weight issues to live healthier and happier lives.
The platform is an NHS-approved diabetes management programme, helping more than three million people in the UK manage their condition and improve their health and wellbeing.
Changing Health’s award-winning behaviour change programme is clinically proven to reduce HbA1c and weight for people diagnosed with type 2 diabetes – and for people who are diagnosed as ‘pre-diabetic’ and was acknowledged by judges in the 2019 QiC Diabetes Awards to be “the future of diabetes prevention”.
“I lost 11kg and 5% body fat in 12 weeks. It’s an enormous buzz when you jump on the scales and see you’ve lost half a kilo in a week. This has changed my life forever.”
“I put my Type 2 diabetes into remission in six months. When I was low, I called my coach and beat myself up, but she looks at the big picture. She’s brilliant.”
“Before Changing Health I had to take a lot of medication to manage my diabetes, which made me feel terrible. I didn't know I had the potential to take control of my health.”
“Since I've got more energy, I'm cooking more at home, running around, doing bits I used to leave for the weekend.”
